Maloniai prašome patvirtinti savo dalyvavimą šventėje iki birželio 1 d. užpildant šią anketą. Maloniai prašome KIEKVIENĄ svečią užpildyti formą, nepriklausomai nuo to, ar atvykstate vienas, ar su šeima. Kiekvienas asmuo turėtų užpildyti atskirą anketą – tai padės mums sklandžiau koordinuoti organizacinius ir maitinimo klausimus. Joje taip pat reikės nurodyti, ar Jums reikės nakvynės šventės dieną, pasirinkti pagrindinį patiekalą, pateikti informaciją apie alergijas ir pridėti kitus komentarus.

Each person should complete a separate form – this will help us better coordinate organizational and catering arrangements. You will also need to indicate whether you require accommodation on the event day, select your main course, provide information about any allergies, and add any additional comments.
